Services
The Alzheimer's Association, Central Illinois Chapter, offers
many services and programs to help improve the quality of life
for more than 21,500 individuals with dementia in our 20-county
service territory and for their caregivers and families.
Agency services include Education, Safe Return, Care Consultations, Information and Referral, and Support Groups.
Care Consultation is a grouping of services to assist the person with the disease and/or their families in planning for and
dealing with all aspects of the illness experience. Components of Care Consultation include: assessment of needs, assistance with planning and problem solving, (which may include the development of an action plan), and provision of support. Aspects of Care Consultation may include: providing information and making appropriate referrals, supportive listening, and follow-up (as needed). Mechanisms of service delivery can be via telephone, email, in-person contact.
Educational programs equip learners with the knowledge and skills needed to enhance the quality of life of the individual with dementia. Learners can include persons with AD, family members, para-professional and professional caregivers,
service and care providers in acute, long term and home and community-based programs, general community, etc. Educational programs can be delivered through: conferences and workshops for caregivers, Activity-Based Alzheimer's Care (ABAC), in-service training for professionals, newsletters, community presentations utilizing speakers bureau volunteers, video library.
Information and Referral services provide information about AD, services provided by the chapter and community resources as they relate to AD. Mechanisms of service delivery must include: Helpline, and can include Contact Center, newsletters, information packets, websites, brochures, resource library.
The Medic Alert + Safe Return Program is an
identification program that assists in the safe and timely return
of individuals with Alzheimers disease or related disorders
who wander and become lost. Safe Return provides family support,
program promotion, on-going educational opportunities to emergency
personnel, as well the lay community on wandering awareness,
education and training
Support groups are regularly scheduled in-person or virtual gatherings of persons with AD, family, friends or caregivers who interact around issues relating to AD. Groups can have social, educational and/or support components and are facilitated by trained individuals.
